REQUIRED DOCUMENTS for Registration with South Hills Youth Football and Cheer:
For the 2020 Season, we are required to collect copies of the following documents:
BIRTH CERTIFICATE - an original must be presented for inspection but a copy will need to be left on file in the team books. If you have participated with us previously, we do not need another copy.
REPORT CARD - we will need a copy of the most recently available report card showing the name of the athlete and the school they are attending. You may choose to use a sharpie or any other method to obstruct data such as grades, comments, etc. if you choose.
We will not hand out equipment until this documentation is turned in.
