2.  Player Birth Certificate

3.  3 Proofs of Residency  (read below for acceptable forms) 

Sidewinder Recommends the Following:

1.  Driver License or Government Issued ID

2.  Utility Bill  --  Water/Gas/Electric   -- showing Current Date,  Full Name,  and Address.
               (other information can be covered)

3.  Mortgage Bill or Rental Payment   -- showing Current Date,  Full Name,  and Address.
              (other information can be covered)
