-Online Registration available at urasports.com. Go to black tab on top right of website and click "Register". It will take you a few steps to register a profile and you can pay via cc.

-Tournament Roster Form (including all players’ full names and birthdates and all managers’ and coaches’ full names, phone numbers and email addresses).
-Copy of birth certificates for each player (player cards also accepted).
-Completed Waiver and Release of Liability form for each player (attached).
-Completed Medical Release/Consent for Treatment form for each player, to be found at http://www.baberuthleague.org/media/20679/Consent_single_writable.pdf
-Little League version also accepted

WAIVER AND RELEASE OF LIABILITY FOR PARTICIPATION
IN THE 2020 URA SUMMER SLAM TOURNAMENT
In consideration of being allowed to participate in baseball tournaments sponsored by the Unionville Recreation Association, and any related events and activities, and intending to be legally bound, the undersigned:
Agree(s) that the parent(s) and/or legal guardian(s) will inspect the facilities and equipment to be used, or will instruct the minor participant to inspect the facilities and equipment to be used, and if the parent or participant believes anything to be unsafe, he/they will immediately advise his coach of such condition and refuse to participate.
Acknowledge and fully understand that each participant will be engaging in sports activities that could result in injury by his action, inaction or negligence, the action, inaction or negligence of others, the rules of play, the condition of the premises, and/or the condition of any equipment used.
Assume all of the aforementioned risk, and accept personal responsibility for any and all damage following any such injury.
Release, waive, discharge and covenant not to sue Unionville Recreation Association, its respective administrators, directors, agents, coaches and other members of the organization, hereafter referred to as “releasees”, from any and all liability to each of the undersigned, his or her heirs, and next of kin, for any and all claims, demands, losses or damages on account of bodily injury or damage to property, caused or alleged to have been caused, in whole or in part, by the negligence of the releasees or otherwise.
We have read the above and release, understand that we have given up substantial rights by accepting it, and sign it voluntarily.
