• Payment: Cash, check, or credit card (if not already paid online)

• Forms:

• Medical Release

• Photo Release

• PCA Parent Pledge

• Concussion Prevention Form

• Birth Certificate: Original (for verification) and a copy for our records

• Proof of Residency: Copies of 3 documents dated Nov 1, 2024 – Feb 5, 2025

(Birth Cert and Proof of Residency NOT required if previously verified as an all-star)
