2018 Umpire Request Forms



2018 Little League Tournament Umpire Request Forms are now available!

Please download from the following link:

2018 Umpire Request Form

Completed forms must be submitted for approval by your District Administrator and received by the Southwest Regional office no later than November 1, 2018.

Forms must be submitted in their entirety for consideration. Please don't forget to attach your current driver's license and 2018 Volunteer Application (included in download form).

Completed forms may be emailed to [email protected] or faxed to (254) 757-0519.
