First Paperwork Turn-in Day

Location: Santee City Hall - Room 8P10601 Magnolia Avenue Santee, CA 92071

Date: Saturday, June 17, 2023

Time: 9am - 11am

What you need to BRING:
1) Copy of BIRTH CERTIFICATE
2) Player Contract (on 2023 Form) 2023 Player/Participant Contract
3) Medical History (on 2023 FORM)
4) Physical (on 2023 FORM)
5) FINAL Report Card for the 2022-2023 school year. (If your child did not attend school, please let the certification rep know)
