FINAL PAPERWORK TURN-IN DAY (MANDATORY)

Time: 9am - 11am
What you need to BRING:
1) Copy of BIRTH CERTIFICATE
2) Player Contract (on 2023 Form) 2023 Player/Participant Contract
3) Medical History (on 2023 FORM)
4) Physical (on 2023 FORM)
5) TWO COPIES of FINAL Report Card for the 2022-2023 school year. (If your child did not attend school, please let the certification rep know)
