Address verification (1 from each section of the residency requirements)
Medical Release Form
Please be sure ALL paperwork is filled out PRIOR to getting in line for sizing. Should you need help with the registration process, please speak to someone at Station 1. We will also have printed forms onsite if you are unable to print at home.
