Tournament Request Form

If you answer yes to one of the 4 items listed below completion of the forms attached are necessary for your team.

Attend a 3rd Tournament within one season
Attend a 4th Tournament within one season
Attend an “Out of Season" Tournament
Attend Other Sanctioned Tournament (ie: US Club)

If your team has answered yes to one of the above items the Head Coach or Team Manager is required to fill out NCFC Youth’s Additional Tournament Request form provided on the link at the end of the page. The team’s parents will be asked to sign the Parental Agreement to participate in Additional Tournament(s) form as well.

Prior to applying to your additional tournament, this completed form must be submitted to your Challenge Administrator and be approved. Your request should be submitted at least 10 days prior to the registration deadline for the tournament to your Challenge Administrator. You will receive notification via email within 2 business days of your approval or denial of your tournament request.

Do NOT register for your additional tournament without prior approval from the Challenge Director.

Note per Challenge Guidelines:

Third Tournament

Third tournament requires 75% roster commitment
Examples - your team has a roster of:
10 players: 75% commitment will be 8 signatures of commitment.
15 players: 75% commitment will be 11 signatures of commitment.

Fourth Tournament

Fourth tournament requires 100% roster commitment

Guest Players

Only parents of your current NCFC Youth Challenge team roster will be signing this participation form. No Guest players are to be included on the signatures.

Off/Out of Season Tournament

This would be any event that occurs during a team’s dormant season. An Off/Out of Season event must have prior approval before registering for this event.

US Club Tournament

NCFC Youth Challenge teams are registered as USYS teams through NCYSA. NCFC Youth registration fees cover the cost of that registration. If a team is applying to attend a tournament that is not USYS sanctioned and only sanctioned as US Club, the team’s NCYSA cards, rosters and medical waiver forms CANNOT be used at such an event. Therefore, the team would have to be registered with US Club which entails an additional registration and set of paperwork as well as a deadline in the NCFC Youth office for the Administrators to be able to complete the paperwork process and allow time to get each player carded for US Club.
