Registration can be completed 100% online. Because you may not have all the forms needed when registering online, you'll have the option to provide the necessary forms via e-mail or in person.
Please submit the following forms/paperwork to the NYSA Certification Committee. Your child will NOT be able to participate until all paperwork has been turned in. You can upload during the online registration process, e-mail to [email protected] or drop them off in person. Follow us on social media so you know what dates are available for in person drop off.
REQUIRED PAPERWORK:
1. Birth Certificate - A clear (COPY) with the official state seal.
2. Report Card - Final Report Card (COPY). ***If child transferred during the school year, you will have to contact the previous school so that all grading periods are on file***
3. Medical/Physical - You will need to go to your physician for sign-off on the medical form. Make sure it's filled out completely with height, weight and blood pressure. Physicals must be dated after January 1st.
