-Player contract form
-Medical release form (if physical dated before 1/1/22)
-Copy of Last Physical (if dated after 1/1/22)
-Copy of Birth Certificate
-Copy of Latest Report Card
-Recent School Photo
-Volunteer deposit $40 (refunded upon meeting volunteer requirements)
Flag Football/Cub Cheer- $100.00
Football (tackle)/Cheer (competition)- $240.00 -includes jersey/uniform
