The State of Illinois and the Illinois Youth Soccer Association (IYSA) is requiring all players to fill out and submit the following forms before any player can participate in training this summer and beyond.
Step 1:
Please download the following forms and fill out each one completely. Please make sure you choose the correct league form that is labeled BOY or GIRL, depending on the gender of your player. In addition to the forms, you will need to submit a current PHOTO a nd proof of age of your player. Once you register, you will receive an email with a link to Google Form to upload all the required forms. Below is the list of everything that needs to be submitted back to us:

- Picture of the player (You provide)
- Picture of birth certificate or passport (You provide)
- League form (Link below)

Step 2:
- Once you have filled out and gathered all required documents, please upload all documents to the Google Form links that were emailed to you recently.
- When you open that email, find your player's gender and birth year and click on that link.
- Once you open the link the appropriate Google Form, fill out all information and upload all documents in the corresponding sections of the form.
