Sports Physical Form

Print out the Sports Physical form, fill it out and take it in to see your pediatrician or family physician. Urgent care centers will also perform sports physicals.

Sports Physical Form

If you just had a Sports Physical performed for a Fall or Winter sport, you do NOT need to have a new physical done for a Spring sport.

Your Sports Physical is for the school year starting May 1 and ending the following June 30.
e.g. May 1, 2010 - June 30, 2011
