Player Requirements to Play

Sports Physical - A sports physical must be performed for a player to participate in off-season workouts, tryouts, and the regular season on all sports teams.

You may have this done at your regular Pediatrician or a local urgent care center. Urgent care centers typically do this for $30-40.

Download and print the Sports Physical Form.

Concussion Baseline Test - A concussion baseline test must be performed at the high school. Contact the Athletic Trainer Mrs. Breanne Piatt to schedule this. It only takes a few minutes.

Cleats & Shin-guards - All players must bring these to every practice and game.
