Sign-ups final Round

We will be there between 6pm and 7:45pm. Make sure to bring a copy of Birth Certificate, Player contract, a wallet sized picture, and payment.

  1. Birth Certificate
  2. Outstanding balance on fees
  3. Final Report Card from the School
  4. Wallet Size Picture
  5. Medical form
