Cost Per Team: $250 (Payable: Murphysboro Soccer, Inc.)

Roster Form: Word PDF 

Co-op Form: Word PDF (completed for any player not enrolled in the primary school district e.g. a Trico player playing for Murphysboro, home school, private school players, etc.
