REGISTRATION
Step #1: From the Affinity Support website, click the "Submit a Request" button at the bottom of the page to submit an upload for processing. If you need additional support, please call the Affinity Help Desk (855) 703-2564. Please make sure to complete the Player Template and Team Official Template following these instructions.
*For current travel players, make sure player names and DOB's are uploaded EXACTLY how they are listed in the database. Otherwise duplicate players will be created and birth certificates will not carry over.
Step #2: VYSA will send the club or league an invoice for the player registration fee and team official registration fee.
Player Fee - $8
Team Official Fee - $20
Adult Players - $22
MEMBER BENEFITS
All registered Virginia Futsal/US Futsal players and team officials receive the following benefits during Futsal activities:
1. Liability coverage
2. Secondary Medical Coverage: Email [email protected] to request the Medical Claim Form
3. Club D & O Insurance
4. Certificate of Insurance. Submit COI request to Kristen (include Name and Address of Certificate Holder)
Player Registration Cancellation and Refund Policy
No refunds will be allowed for registration fees associated with players or team officials once the team and/or additional players are activated on the roster.
