Clark Youth Football Registration

REGISTRATION FORMS & INSTRUCTIONS  
(Doctor’s form does NOT have to be returned at time of registration)

You can begin registering for the Fall 2019 season on August 20, 2018.

In addition to online registration, all forms must be returned prior to the first practice: any child without this form by that date will NOT be permitted to practice or play until hard copies of the forms are submitted.
 
All payments must be made at the time of registration. 

Clark Youth Football is open to the residents of Clark and Garwood only, as per our league rules.
