Player Attestation (Grades 6 -12 Only)

Camas School requires all participants in Grades 6 thru 12 complete a COVID-19 attestation before using a CSD facility or field.  Camas Little League uses several school fields for baseball / softball practices and games.  To be compliant with this requirement CLL has created a player attestation form that each player (or that player's parent/guardian) must fill out each day when participating at a CSD field.  Fields affected include Liberty A and Liberty B for softball and Liberty C for baseball.

The attestation form includes the following questions:

• Does your child have any of the following symptoms within the last day that are not caused by another condition? Cough, shortness of breath or difficulty breathing, fever of 100.4°F or higher, sore throat, headache, chills, new loss of taste or smell, muscle or body aches, nausea/vomiting/diarrhea, congestion/running nose, or unusual fatigue

• Has your child been in close contact with anyone with confirmed COVID-19?

• Has your child had a positive COVID-19 test for active virus in the past 10 days?

• Within the past 14 days, has a public health or medical professional told your child to self-monitor, self-isolate, or self-quarantine because of concerns about COVID-19 infection?

• Has your child had any medication to reduce a fever before coming to school grounds?

If a participant answers YES to any of the above questions, they are not allowed to enter school grounds.
