In order to play Sports for St. Michael School, you must be in 5th through 8th grades, AND either be a St. Michael School student or enrolled in the Faith Formation program at St. Michael Parish.

The NWI Catholic Youth Organization (CYO) also requires that you complete three forms in their entirety.

The first is a CYO registration form which includes a Sports Physical section. The form can be found by clicking this link.

The second is a Sportsmanship agreement. This must be signed by the player and the parent. The form can be found by clicking this link.

The third form is an acknowledgment that you have read the information on concussions and sudden cardiac arrest. The form can be found by clicking this link.

Once these forms are completed, then you can register and pay to play sports for SMS. These forms only need to be supplied once per year. Once a student has provided these forms, they are eligible for 12 months.
