The 2023 season is approaching quickly.

Verification Dates:
12/14/22 at Claim Jumper Restaurant in Rancho Cucamonga 6:30PM - 8:30PM
1/4/23 at Logan’s Roadhouse in Fontana 6:30PM - 8:30PM
1/7/23 at Coyote Canyon Ballpark 9:00AM – 2:00PM
1/11/23 at Logan’s Roadhouse in Fontana 6:30PM - 8:30PM

What you need for our mandatory verification dates:
Bring forms even if you uploaded them to our website

• Birth Certificate
• Medical Release Form
• Concussion Baseline Form
• Concussion Contract
• Covid Release Form
• Age Verification – see attached form: Little League Residency and School Attendance Eligibility Checklist
• Address Verification – see attached form: Little League Residency and School Attendance Eligibility Checklist
• School Enrollment Form – see attached form: Little League Residency and School Attendance Eligibility Checklist

At verification we will only be able to verify players that have completed the online registration.  It is very important that before attending any of our verification dates that you have successfully registered online through our website registration portal.  If you have questions about our online registration, board members will be present to assist you.  Please remember that we will not be taking any payment in person; all payments for registration must be made online through the registration portal only.  

We ask that you upload your verification forms on the registration portal and bring them to the verification date.  All forms MUST be originals (no copies) and we need to be able to keep them if your player/s are eligible for All-Stars.  Minor and Major parents will be asked to sign an All-Star commitment form. 

Please bring all registered players to verification for uniform sizing and information.
