We’re excited to have you join us for the 2026 season.

Before you begin, please review the items below so you’re prepared to complete your registration in one sitting.

Registration Requirements

Registration is not considered complete until all required items are submitted.

At Time of Registration (upload required):
• A clear photo of your child
• A copy of your child’s birth certificate
• Medical consent form

Completed During Registration:
• Medical history form
• Participation agreement &  Parent contract

Required Before the Season Starts:
• Most recent report card
• Physical form completed by a doctor

Please make sure all information is accurate and all documents are ready before starting. This will help avoid delays in your athlete’s eligibility.
