DOCUMENT VERIFICATION January 11th!

We will be doing document verification this Friday, January11, from 6-8pm during makeup evaluations at Tijeras Creek.

All players must have their documents verified in order to play in the Spring 2019 season.

Every player, new and returning, must show proof of residency. Original Birth Certificates PLUS one form of documentation from each of the 3 groups must be presented to the SMLL verification staff in order to be eligible to participate in the Spring 2019 Season.

SMLL does not keep any records on file so copies are not required. The 3 groups are:

Group 1
1. Drivers License; 2. School records; 3. Vehicle records (i.e. registration, lease, etc); 4. Employment records; 5. Insurance records

Group 2
1. Welfare.child care records; 2.Federal records (federal tax, Social Security, etc); 3. State records; 4. Local (municipal) records; 5. Support payment records; 6. Homeowner or tenant records; 7. Military records

Group 3
1. Voter’s Registration; 2. Utility bills (i.e. gas, electric, water/sewer, phone, mobile phone, heating, waste disposal); 3. Financial records (i.e. loan, credit, investments, etc); 4. Medical records; 5. Internet, cable or satellite records
