Below please find the links to the 2018 Pop Warner Volunteer Application and the 2018 Clay PAL Supplemental Volunteer Application. Please be advised that the forms must be filled out COMPLETELY and TRUTHFULLY.  If you are not sure of what roles are available for the team you are interested in helping with, you can leave that section blank for now but please complete all of the other sections.  The second page of the Supplemental Application must be signed in the presence of a Florida Notary.  

Please return BOTH of the completed forms, along with a copy of your current, valid Driver's License or State Issued ID, to [email protected].  You will also need to bring the forms with you to the Parent Meeting.
