3.     Physical Form, We will have on site. You'll need to get your Doctor to perform the physical, fill out the form and sign.

4.     Original Birth Certificate with the raised seal, for new sign ups. Returners we have on record.
5.     Current School year Report Card. 2016-2017 School Year (FULL YEAR)
