Please bring the following forms to the first day of practice:

2019 Pop Warner Medical History and Health Form
NJ state code-of-conduct.pdf

Please also bring:
1) Copy of his/her Birth Certificate or Passport.  Originals will be verified when proceeding to Cheer Competitions.
2) Copy of your child's final year-end report card from the most recent school year.
3) Recent 2x2 head shot picture of your child.
