We have provided for your convenience a checklist of all the forms needed to fully register for our 2018 season. All documents stated below should be returned to the Team Administrator on or before Monday, August 27, 2018. However, you may return each of the documents as they are completed.

** Physicals are due the first day of practice (no later than August 27th). Participants will not be allowed to practice without it.
Following documentation should be submitted to the Team Administrator.
Birth Certificate (Copy Only)
MVA Identification Card (Can be obtained from any MVA Office)
Report Card (must contain all 4 marking periods for 2018 - 2019)
