School Enrollment Form
Need form is needed if you don't live in district but go to one of the following Schools:

• Douma Elementary School
• Liberty Elementary School
• Evans Middle School
• Ottumwa Christian School

Volunteer Application
This form will need to be filled out by any Board Member, Manager, Coach, Parent who would like to be involved with our league.
