MMYFLC ID’s will be made for football and Cheer participants on Thursday, August 23, 5:30-7PM at Howard County Recreation and Parks, 7120 Oakland Mills Rd, Columbia, MD 21046.
Parents will need:
- Your child’s original birth certificate or passport. If you don’t have the original they must in addition to the copy of the birth certificate or passport bring two official documents that have child’s name and date of birth (insurance card, school record, medical record, etc)
- The kids must be present, they will be photographed onsite. You do not need to bring any pictures.
- $30 cash only
- The form above filled out in pen or typed.
The ID’s are good for 2 years. If someone has an ID from last year and it says 2017-18 it is valid for this year. If the ID says 2016-17 it is not valid for this year. Required documents for the ID are listed on the top of the form.
We will also make MMYFCL ID’s at the league meetings at Long Reach High School Sunday August 26th from 4:30-6pm
