According to the Pennsylvania Department of Human Services, all prospective volunteers MUST obtain the
following certifications:
1. Report of criminal history from the Pennsylvania State Police (PSP).
2. Child Abuse History Certification from the Department of Human Services.
3. A signed Disclosure Statement affirming in writing that they are not disqualified from
volunteer service based upon a conviction of an offense. (see attached form)
4. A fingerprint based federal criminal history (FBI) submitted through the Pennsylvania
State Police or its authorized agent is required if:
*The volunteer has lived outside the Commonwealth of Pennsylvania in the last
10 years.
You can obtain these clearances by following these easy steps:
1.
PSP criminal history can be accessed by going to:
https://epath.state.pa.us//Home.jsp b. Read and accept the acknowledgement
c. Complete the form
d. Volunteer Organization: “name of local organization”
e. Print out results of the Search
f. Bring results to next meeting.

The Child Abuse certification can be accessed by going to:
https://www.compass.state.pa.us/CWIS b. Create an individual account and a temporary ID/password.
c. Receive an email with a temporary password, log into the
account and create a permanent Keystone ID and password.
d. SAVE this LOG IN INFORMATION-you will need it.
e. Accept and acknowledge the terms.
f. Complete the form online.
g. Select Volunteer Status.
h. Type in “name of local organization”
i. Print out the certificate
j. Bring certificate to the next meeting.
3. Sign form given to you by your league.
