2018 Player Registration

2018 Physical Form - Dated after January 1st of this year and signed and stamped by a physician. Player will not practice or receive gear until this is on file!
2017-2018 Year-End report card (showing all quarters/semesters).
Copy of original birth certificate. Must be state issued, not a hospital certificate.
Player photo - wallet or passport sized.
$175 includes a $25 late fee (the deadline has past)
