Each coach must maintain a complete team book that contains all of the following information.

Required for all coaches and assistants

• NAYS.org Training & Insurance
• Child Abuse Recognition training
• Concussion training
• Coaches application & background check

Required for all parents

• Code of Conduct form

Required for each player

• Birth certificate (copies are ok)
• Registration form
• Medical release form

When building your book, we recommend using a letter sheet protector for each player and coach to hold all of their information together.

The order of the book will go as follows:

1. Team Roster (5 copies are required)
   1. 5 copies
   2. Alphabetical order by last name
   3. League name - "Paulding County"
   4. Select both age groups
   5. Use player names as they are on the birth certificate and not the player registration form

   2. Birth certificate facing backward
   3. Medical Release Form, Parent Code of Conducts in the middle
