VOLUNTEER FORMS

Returning Coaches please complete the forms and bring a copy of your driver's license  to our Volunteer Coordinator. You may email the forms TO [email protected]or drop them off in concession.

VOLUNTEER REQUIREMENTS & INFORMATION

Please see the following instructions to become a Coach or Team Mom in Football or Cheer.
  • All Head Coaches are Appointed and Voted Upon by OSA Board of Directors.
  • Team Staff Members are selected by the Head Coach and approved by the OSA Board
  • All Volunteer Paperwork Must Be Submitted by deadlines issued for each sport you are wishing to volunteer for. 
  • Volunteer Paperwork may be emailed to [email protected]
  • All Badge Process Documentation and Online Courses must be completed by June 30th for Pop Warner coaches in order to have your badge ready for August 1st.

NEW VOLUNTEERS TO OSA POP WARNER & CITY OF JAX PARKS DEPT
  • All New Volunteer must complete the Pop Warner Volunteer Form. You may also be asked to complete an OSA Volunteer Application Form and/or Coaches Agreement Forms.
  • All Volunteers MUST provide a CLEAR copy of your Florida State Drivers License.
  • Football Volunteers- Must complete the 4 hour Pop Warner Coaches Clinic provided by Greater Jax Pop Warner Conference and successfully pass the end of course test . This Clinic must be renewed every 3 years.
  • Football Volunteers- for Tiny Mite must complete the Tiny-Mite certification online by following the link on the www.jaxpopwarner.com.
  • All Football Volunteers must complete USA Football Online Tackle I Football Course. This must be renewed every year. 
  • Cheer Volunteers– Must complete local Pop Warner All-Day Cheer Clinic provided by Greater Jax Pop Warner Conference. This clinic certification is good for 3 years.
  • All Cheer Volunteers must complete YCADA Online Training.  YCADA Certificate is good for 2 years.
  • All New Volunteers MUST complete background check through the City of Jax Parks Dept. –If you have a current badge from the City Parks Dept., please let us know. City Background checks can be done during the Pop Warner clinics at First Coast HS with Mike James, City Athletic Director.
  •  If unable to get in to see MIKE JAMES during Clinic, CALL OUR VOLUNTEER COORDINATOR TO INSTRUCT YOU FURTHER!
  • If you have a break in service(ie Take off a year), you must have your background checked again with the City Parks Dept. if you do not have an active/current badge with City Parks
  • Must complete OSA Coaches Code of Conduct Agreement. (Will be provided at Volunteer Meeting)
  • Must pay all applicable fees to the Association. Those will be determined and you will be notified. 
  • Clinic Fees and Certification Course Fees will be paid when you sign up to attend or log on to sites to complete course.
RETURNING VOLUNTEERS TO OSA / POP WARNER
  • All Returning Volunteers must complete the Pop Warner Volunteer Forms.
  • Must provide a CLEAR copy of your Florida Driver’s License
  • Must check to see if your Clinics are up to date. Clinics are only good for 3 CONSECUTIVE years.
  • Must fill out COJ Continuous Form for Background verification.
  • All Football Volunteers must complete USA Football "Heads Up Tackle 1" Online Course, including Team Moms.
  • Online Course Instructions and Discount Code will be given once you have been determined as a Team Staff Member. 
CHEERLEADING VOLUNTEERS
  • Please fill out Pop Warner Volunteer Forms.
  • Follow instructions above for badge process. 
  • New Coaches, Team Moms, Coach Trainees and Student Demonstrators must attend the Cheer Clinics. Clinic is good for 3 years. Must be renewed every 3 years.
  • Register online for the Cheer Clinic- see Greater Jax website for link to register.  
  • YCADA Online Course - Must be completed by all Coaches, Team Moms, Coach Trainees and Student Demonstrators. Print and submit the End of Course Certificate to [email protected]. Course is good for 3 years.
TEAM INSTRUCTIONS
  • Team Staff is chosen by the Head Coach with Board Approval.
  • Each Team is allowed a maximum of 8 badged staff members
  • Each team must have at least 1 CPR certified staff member. This person must be at practices. Recommend having 2, for a back- up. CPR Person must be a Badged Staff Member on your team roster.
  • Each Team will be provided an Emergency Action Plan.
