Volunteer Form Information

The Little League website has the 2024 volunteer forms available.  
District 7 must run backgrounds on Staff and Presidents every year just like leagues run backgrounds.  There are 2 forms see bottom of page:  1) the form that requires a SS# for new people and 2) the form for returning volunteers - called 'basic'.  I'm assuming everyone uses JDP to run backgrounds.  

General Directions:  Please do the following:

If you are a new President, you will need to fill out the form that asks for a SS#.

If you are a returning President (and all staff), you can fill out the basic form (no SS# required).  You then should receive an email from JDP asking you to complete the background using a link in the email (because your background has previously been done by D7).

1.  Open the form needed.
2.  Fill out the information (can be done online or print the form)
3.  Print the form (I don't believe the program allows you to save the data)
4.  Scan it in (if possible)
5.  Send to Leslie via email

If you can't scan it in, please mail the completed form to:

Forms still need to collected as it gives the District your permission to run the background check - hence still needing the paper copy to be emailed or mailed.
