Ready To Register?

1. Fill Out Some Forms

The following forms need to be filled out:

• Registration Form
• Code of Conduct
• Media Release Form
• Medical Release Form

This can be accomplished by clicking the button below. 

Please note: Each player will need their own completed paperwork. Come back to this step, and and hit the Resubmit button when prompted.

Once you complete the forms, you will be emailed a copy of each form. You will then need to upload those forms in the next step. Please check for any emails from [email protected]. 

2. Continue Your Registration In Sports Connect

You can continue the registration process via Sports Connect by clicking here:

During the registration process, you will be asked to upload a copy of the forms you have completed in the previous step.
