HAMILTON PARK LITTLE LEAGUE

To start registration you will need to create a new account and follow the creating account process. Divisions and prices are listed under the REGISTRATION LISTING section, which is located under this section.

Below please see some “need to know” information to help with a smooth and quick registration process.

  • All players MUST be registered via online through our website.
  • In-Person registration will go faster if players are register online prior to attending in-person registration. 
  • For those unable to register online before the in-person registrations days, we will have stations setup to access online registration.
  • After registering players online the required forms to complete the registration process, medical release form, 3 proofs of residence or School Enrollment form and copy of Birth Certificate must be brought to one of the In-Person registration dates.
  • All required forms can be obtained online.
    • Forms like the Medical Release and School Enrollment form can be completed prior to attending the In-Person registration dates for faster quicker process.
After registering each child the following items will be required to finalize registration for each child.

(1) Complete and return a copy of Medical Release form. The form can be obtained by clicking the link below.

(2) To verify school/residency is within the Hamilton Park Little League district, please bring a copy of 3 different proofs of residency one or more from EACH group listed below dated between January 31, of previous year and February 1, of current year, OR those players attending school within the boundary can complete a School Enrollment Form, signed by the school's principal or vice principal. The form can be obtained by clicking the link below.
School Enrollment Form:

Group I
1. Driver’s License (photo ID of parent(s) or guardian(s) with qualifying residence address)
2. School records (home address of player’s parents or guardians)
3. Vehicle records (i.e., registration, lease, etc.)
4. Employment records5. Insurance documents (with residence address)

Group II
1. Welfare/child care records 
2. Federal records (i.e., Federal Tax, Social Security, etc.) 
3. State records
4. Local (municipal) records
5. Support payment records
6. Homeowner or tenant records 
7. Military records

Group III
1. Voter’s Registration
2. Utility bills (i.e., - gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
3. Financial records (loan, credit, investments, etc.)
4. Medical records 
5. Internet, cable, or satellite television records

NOTE: Example – Three documents from the same Group (utility bill, cable bill, and bank statement) constitute only ONE document.

(3) Copy of Birth Certificate

The above required documents can be provided via email using the email address listed below. Or please contact our player agent to schedule a time to meet and collect the documents.

All items MUST be provided to finalize registration.


Once online registration has closed, rosters will be determined for Non-Major Division teams and you will be notified by your child's coach.
