The State of Illinois passed a new law concerning Concussion Education for players and parents. In accordance with his new law we are asking all parents and players to read the linked information sheet.

I have reviewed the Parent and Athlete Concussion Information Sheet linked to this registration form and understand the contents and risks thereof.  I understand if my child has signs and symptoms of a concussion, he/she must be removed from the practice or game immediately. He/she will not be allowed to return to play until medically cleared. Medical clearance is not given until a player provides a written note from a licensed medical physician.
