Online Registration Now Opened!!!!

  6. Birth Certificate - the original birth certificate is only required for new players. Once the birth certificate is "certified" by Pop Warner you will receive it back. 
  7. Pop Warner Consent form
  8. Pop Warner 2018 Physical form
  9. Photography Release form
  10. Report card - Elementary students must turn in last quarter report cards. Middle and High school students must turn in 1st and 2nd semester report cards (full report card)
  11. Passport sized photo of your child
  12. The concussion course; Please note, all players must complete the concussion training. Please select the following link and complete the class. Once completed, print the form and bring it to the field to complete your registration. https://www.cdc.gov/headsup/youthsports/training/index.html.
