Pop Warner Covid-19 Disclosure

Pop Warner football and cheerleading requires all participants (players and cheerleaders) to submit the signed Covid-19 disclosure, acknowledgement, and waiver to be completed by all youth athletes, coaches, and volunteers.

Download the waiver here and provide it to the Rams' delegate at practice.
