Soccer Club Staff Resources

Below you will find important information for our staff members.  All staff members and administrator must complete the below "Required Paperwork" information prior to being included in a payroll.

Required Paperwork

  • W-9 Form - Must be completed by all staff members and returned to [email protected] with a signed contract prior to first payroll.
  • Completed Contract - Please contact [email protected] or your Director of Operations.
  • Additional paperwork (Risk Management, Concussion Documentation, Lindsey's Law, USYS Membership Registration Form, Safesport, etc) will be needed and will be coordinated through our club admin and will be collected via e-signature.  Our Club Administrator can be reached be emailing [email protected]  Once all information has been received and your head-shot taken you will be eligible to participate in payroll.


Season Planning for Coaches

We will utilize this form two times per year. Coaches will submit a listing of league and tournament selections for U14 and below teams in the fall and again in the spring. U15 and above teams will be completed in late summer.  This form submission should be done in consultation with the Director of Coaching and Director of Operations.  Club leadership will review your form submission to ensure all selections are within budget constraints.

Fall Season Planning Due by: June 25th
