Required forms for your child to complete after registration. There will be a document-upload email coming out some time after initial registration.
The following forms are not registration forms. They are documentation required by the Chesterfield basketball leagues.
No child will be permitted to play without these completed forms and a copy of your child's birth certificate (previous season players do not need to provide their birth certificate).
