Refund Request Forms

Due to the games being postponed we wanted to provide the opportunity for those that had decided not to participate in the 2020 season to download the refund request form. After filling out the form you can email it to [email protected] or contact 304-888-8484 to make arrangements to drop off. Those requesting a refund should do so in the next two weeks to allow us time to verify registration and process the refund.
