EVERY TEAM MUST CARRY THE FOLLOWING DOCUMENTS TO EVERY GAME:

1. Team Roster signed by the League President.  Team Rosters need to include the following information:
   - Players: First & Last Name, address, league age, birth date 
   - Manager and Coaches’ First & Last Names address, Phone number 
   
   
2.  League ID for All Managers & Coaches Signed by the League President) supported with a government ID upon request.  ONLY MANAGERS AND COACHES LISTED ON THE TEAM ROSTER AND CARRYING A LEAGUE ID WILL BE PERMITTED ON THE FIELD PRIOR TO THE GAME AND IN THE DUGOUT DURING THE GAME. (Maximum 1 Team Manager & 2 Coaches)
   
   
3. Pitch Count Data Sheet (Log) to be submitted to Host League scorekeeper at beginning of each game, & at end of each game by Tournament Director and Manager and carried to next game by team for pitch count verification purposes.
   
   
4.  Medical Release Forms for every rostered player: Use your own league form or this one.
   

COACH TRAINING — COMPLETION OF THE FOLLOWING IS REQUIRED:

1. HeadsUp Coach Training: All Managers & Coaches 
2.  Provide each Parent/Player with associated HeadsUp Fact Sheet
3.  LL Diamond Leader Completion Certificates
4.  SafeSport Abuse Awareness BASE Training Completion Certificate
5.  Sudden Cardiac Arrest Prevention
