REQUIRED DOCUMENTATION
- End of year report card (can be turned in at end of school year)
- Current physical (must be within last 12 months of current date)
- NC State DMV ID (can be obtained at the local DMV. Must have Social Security card, original birth certificate)
- Participant Birth Certificate (NO Original copies will be kept by the associations. Please deliver a photo copy or screen shot of the birth certificate)
- Download and completed the required forms located on our FORMS page

12U (11-12) July 31, 2010 (Can't be 13 on or before July 31st)

How do we determine registration cost: Each year, participant registration cost is determined by our current and projected equipment needs in addition to our fundraising efforts of the prior season.. Parent and player participation at team fundraisers play a huge role in setting registration prices. Please continue to check Facebook, website and coaches for a list of upcoming fundraisers.
