REQUIRED DOCUMENTATION
- End of year report card (can be turned in at end of school year)
- Current physical (must be within last 12 months of current date)
- NC State DMV ID (can be obtained at the local DMV. Must have Social Security card, original birth certificate)
- Participant Birth Certificate (NO Original copies will be kept by the associations. Please deliver a photo copy or screen shot of the birth certificate)
- Medical Sports Physical (Download and complete the required forms located on our FORMS page)

6U (5-6) Birthday has to be BEFORE July 1, 2019 (Can't be 7 on or before July 1st)
8U (7-8) Birthday has to be BEFORE July 1, 2017 (Can't be 9 on or before July 1st)
10U (9-10) Birthday has to be BEFORE July 1, 2015 (Can't be 11 on or before July 1st)
12U (11-12) Birthday has to be BEFORE July 1, 2013 (Can't be 13 on or before July 1st)

How do we determine registration cost: Each year, participant registration cost is determined by inflation, current and projected equipment needs in addition to our fundraising efforts of the prior season.
