We require each player to provide the following documentation prior to taking the field with their respective teams. Team Manager will begin collecting from you in late Spring / early Summer:

1. Adult Code of Conduct
2. Athlete Code of Conduct
3. Player Physical signed by Doctor dated after January 1, 2023 & before participation
4. PRESENT ORIGINAL BIRTH CERTIFICATE; leave a copy with Team Manager
5. REPORT CARD with all four quarters of prior school year - please provide a hard copy 
   
6. 2" x 2" passport like CURRENT HEAD SHOT PHOTO; shoulders up
