All registered Brookfield Little League players must live or attend school within the charter boundaries (Brookfield,IL) 
3 proofs of residency or a completed School Enrollment Form are required prior to team placement


SCHOOL ENROLLMENT

If your player attends a school in Brookfield (Congress Park, Hollywood Elementary, Lincoln, SE Gross or St. Paul's Lutheran School), you may complete and submit a School Enrollment Form in lieu of 3 proofs of residency.

Please note that even though Brookfield residents attend Brook Park Elementary, the school's physical location is LaGrange Park. Therefore, the School Enrollment Form is not applicable.

Please follow the directions on the form and ensure that it is signed by the school principal or authorized administrator.
