T-Mobile Call Up Grant

What is the T-Mobile Call Up Grant Program?

The T-Mobile Little League Call Up Grant Program is committed to helping families in need by covering registration fees associated with local Little League Baseball or Softball so that every kid has the chance to play. Little League and T-Mobile share the belief that every kid should have the chance to experience Little League Baseball and Softball, regardless of their financial or personal situation.

This is not a competition or a chance to win; everyone who meets the requirements can receive assistance - until funds run out, so act quickly!

This program is available to families in need who can present verification that shows they are currently enrolled in one of the following programs:

Headstart: 
Headstart participants are eligible and need to show that they are currently enrolled. Contact us for more information.

Free/Reduced Lunch Verification: Yes, even with the current USDA subsidy, Skyward will still show this information, but you must have a way to show that it's from this school year or is current.  (They are looking for evidence that it is not a screen print from last year.)

  • BEST OPTION: Copy of an email or picture of a letter with a date showing your player qualifies for free or reduced lunch for this school year or with the current date. (example)
  • MIGHT WORK: Screenshot from Skyward showing the Free or Reduced Lunch eligibility AND has recent activity shown underneath it so you can tell the screen print is not from last year.  This typically works if you are submitting some payments (reduced lunch, second serving, cash bar, etc). (example)

SNAP or WIC: Need to show that you are receiving benefits currently.

  • BEST OPTION: State of Indiana Proof of Eligibility Form 53549: On the FSSA Benefits Portal (link)login, and then go to "My Account", "Cases/Forms" (top option), and look for "Proof of coverage". (example)
  • ALSO WORKS: WIC smartphone app screen prints as shown here
  • MIGHT WORK: Copy of a letter or email stating that you are eligible to receive benefits, including date (example)


Hoosier Healthwise or Medicaid or State Health Plans: 
 Proof of enrollment must include something showing the data.  The front and back of the card will no longer be sufficient.  We recommend:

  • BEST OPTION: State of Indiana Proof of Eligibility Form 53549: On the FSSA Benefits Portal (link), login, and then go to "My Account", "Cases/Forms" (top option), and look for "Proof of coverage". (example)
  • MIGHT WORK: Documentation from your carrier's website showing proof of coverage that shows it is currently active (the dates). The links are:  AnthemCareSourceMDwise, and MHS.
  • MIGHT WORK: submit an email or a picture of a physical letter showing that your benefits were approved for the current year.

 

Foster Child documentation with player name and a date showing they are legal foster children.


TIP: If none of these documents are available, contact us and we can help you find alternatives!

 

How do I participate?

If you have not already registered:

  • Complete the registration.   When asked for payment, change the payment type (drop-down box) from "Credit Card" to "Check".  The system will finalize your registration without payment.
  • AS SOON AS POSSIBLE, please send a picture of your proof of eligibility (from the list above) to [email protected].  This information is only seen by one administrator, is kept confidential, and is not shared  with other Board members, coaches, etc.
  • We will complete the paperwork with T-Mobile on your behalf.  When we receive the check from T-Mobile, we will apply the payment.

If you have already registered and paid in full:

  • Contact us and we can help you get a refund!
