For each athlete to participate all Registration Fees must be paid in full by May 19th.  Copies of the following documents MUST be completed fully and turned in at a Sizing Date (NO emailed or uploaded documents will be accepted) :

• A Copy of athlete's birth certificate
• A Copy of proof of grade Tackle Football & Cheer Only - (report card, ParentVue, etc.)
• A Copy of the athlete's 2023 Physical on file (must be dated 4/15/2023 or later).   Most physicians have these forms on file - or CLICK HERE for the current Michigan High School Athletic Association Physical Form.
• Concussion Form, Code of Conduct Form and all other required forms will be available at sizing dates.
