2024-2025 YOUTH PARTICIPANT MEDICAL HISTORY FORM

For the Physical Fitness and Medical History Form:

Section I (Page 1) must be returned by everyone.
Section II (Page 2) may be substituted with a 2024 NYS School Health Examination Form.

While OTP understands that some children have not yet had their annual physical, Pop Warner requires this paperwork to be dated on or after 1/1/2024.
