Pitcher Eligibility Tracking Form*
*This form is REQUIRED for all player pitch baseball divisions (Minors, Majors, 50/70, Junior and Senior). This form MUST be filled out in PEN and have the required signatures. This is a working document and MUST be kept all season long and be provided at every game. If a new form is needed, simply staple a new sheet to the old one. If a team is found not to have this, it may result in the suspension of the team's manager.

Baseball Pitch Log**
**This form is NOT REQUIRED and is used to assist the scorekeeper(s) of pitches thrown. This is not a suitable replacement to the Pitcher Eligibility Tracking Form
